Super cute dark gray with black polka dot ruffle dress by OH Baby and Motherhood. Lightweight, has a cute ruffle that carries down from the neck to the bottom of the skirt. It does have two small snags on the left side of the skirt toward the bottom shown in the 3rd photo. No belt, but it used to have one. It's not needed.
